Use this form to request any individual review task or to build your own review (e.g., a Student Review and a copyedit, just a copyedit, just a Standards and Student Review, or a complete, full review).
(For an initial review of a new or redeveloped course, please select "Comprehensive Review" instead.)
Use this form to request federal loans from the FAFSA be re-offered if a student has previously declined their loan(s) and the loan(s) are no longer showing on the Financial Aid portal.
👀 ONLY COMPLETE THIS FORM IF THE FOLLOWING APPLY (check Advisor Assist to confirm):
✔ Student has completed a current FAFSA.
✔ Student has actually declined federal student loans for the selected term(s).
✔ Student is NOT on Satisfactory Academic Progress (SAP) Suspension.
✔ Student is currently or will be enrolled in AT LEAST 6 program applicable credits for the selected term(s).
Please submit only one form and select all terms which the student would like loans re-offered.
