BEFORE CLICKING "REQUEST SERVICE," PLEASE READ ALL OF THE FOLLOWING:

Why Should I Use This Form and For What?

This form should be used any time an employee needs/wants to change offices. By filling it out and submitting it, all of the involved support teams will be able to better serve and address any needs during the move process. During the first two weeks and last two weeks of any semester, ALL moves are put on hold!

Instructions:

  1. After reading through the "What if..." questions below, click the green "Request Service" button.
  2. Read all the fields and their help prompts on the new page that opens.
  3. Fill out the necessary fields and click "Request."
  4. After the move request is submitted and approved, you will be contacted by the applicable support teams within two business days to further coordinate the move.
    • What if...I need help finding additional space?
      • Please call Chad Price ext.1260 before submitting the request.
    • What if...I have a move that will affect a ward computer?
      • Please include that in the "additional comments" section of this form.
    • What if...I have been told that a person/people need to temporarily relocate?
      • Please contact the Technology Moves Coordinator at technologymoves@byui.edu to discuss how large scale it will be.
    • What if...I have an employee changing departments? What happens to their technology?
      • Please contact your department’s Technology Support Coordinator (TSC)* to discuss what will happen with the computer since they are purchased by departments (not necessarily for individuals).
    • What if...I have a new hire starting or a retiree/employee leaving? What should I do about their technology?
      • Please contact your department’s Technology Support Coordinator (TSC)* to discuss what will happen with the computer setup/takedown.
    • What if...I need to request a phone or change an extension?
    • **What if...I'm unsure who my TSC is?**
      1. Please call x.1411 and follow the prompts to reach Technology Support. They can look up your TSC.
    • What if...I still have move related questions?
