This application notifies BYU-Idaho of your intent to complete all requirements for a degree or certificate within the current term or the next term. Please do not submit this application until you are registered for all remaining courses.

If you were awarded a certificate or degree last semester and have not received a physical copy of your certificate or diploma, please email graduation@byui.edu. DO NOT use this form to request a lost, damaged, or duplicate copy of your credential.
